Health Risk Questionnaire (HRQ) Information
Your recent Open Enrollment material and DVD told you to watch for new features in your New Indemnity Medical Plan.  We are now implementing a new program to help improve your health, well-being and quality of life.  We strongly believe that investing in the health of our participants provides long-term payoffs for you, your family, and the Benefit Fund.

The new health benefits include:

•       a confidential Health Risk Questionnaire (HRQ)
•       a personal and confidential results booklet
•       a Financial Incentive in your Health Reimbursement Account (HRA) for completion of the Health Risk Questionnaire

The program is voluntary, confidential and is included as part of the New Indemnity Medical Plan.  The Benefit Fund has contracted with StayWell to administer the program.  StayWell is an independent company that has been chosen based on its proven ability to provide quality health risk questionnaires.

It’s Confidential.  The HRQ is confidential and no individual health or personal information will be provided to the Benefit Fund, your Employer or your Local Union.  Your health and personal information will be used by StayWell to provide you with your own confidential report.  As part of this program, StayWell may also provide your Personal Information to the Benefit Fund’s designated health care contractors so that they may respond to your inquiries and provide you with other health-related services.  Your information will also be used at the group level to help the Benefit Fund better understand the overall health needs of its participants; your specific health information is not identifiable in any of the aggregate group reports.
